FMCSA opens comments on renewing the CMV Driver Medication Form used at DOT physicals
On September 15, 2026, FMCSA published a Federal Register notice seeking Office of Management and Budget approval to renew an existing information collection: the 391.41 CMV Driver Medication Form. This is a notice, not a rule — it does not change any medical qualification standard. Comments are due by October 15, 2026, under docket FMCSA-2026-0760.
The form is voluntary. It gives a certified medical examiner a standard way to ask a driver's treating healthcare provider about medications that have been prescribed, so the examiner can judge whether a prescription or the underlying condition affects safe driving. In FMCSA's words, the information "assists the ME in determining if the driver is medically qualified" and helps confirm there are no disqualifying medical conditions.
Renewal notices like this one are routine — federal agencies must periodically re-justify the paperwork they ask the public to fill out. The practical point for carriers is that the form is staying in use, which means a medical examiner may still send a driver back to their doctor with it before issuing a medical certificate.
